Healthy Communities, St. Sabina Wellness Center
Contract RN for Wellness Program 
· Facilitate POC testing and biometric screenings
· Review and help participants interpret key data points in the wellness screening (PHA, labs, vitals, recommendations)
· Develop individual coaching plans for improved wellness and support each participants’ chosen path to wellness
· Refer parishioners to FQHC safety net system care providers in the absence of a PCP and assist in securing access to an appropriate physician, and understanding medical benefits
· Support implementation of the wellness portal Encourage You by assisting participants create individual accounts 

·         Part Time Contract work for Cerner’s St. Sabina Clinic (Belton, MO)
.         $30 p/hr
·         Hours are super flexible, work out with Director
·         20 hours per week
·         Monday – Friday only, no weekends
         8:30 – 1 pm for screenings
·         Must speak Spanish
         Charting is in Cerner’s Power Chart System
·         Wants to start someone ASAP




SHS Wyandotte Clinic is seeking a Licensed Practical Nurse. The LPN will take a health history, perform health counseling, administer medications and treatments, provide nursing care and assist physicians/providers. Additionally, the LPN will follow up on abnormal lab results, abnormal imaging results, post hospitalization, do pre-visit planning, and patient teaching. QUALIFICATIONS:

1. Graduate of an accredited school of practical nursing.
2. Licensed to practice in the State of Kansas.
3. Basic Cardiovascular Life Support (BCLS), certified.
4. Excellent communication skills, customer service oriented, telephone etiquette and exhibit the ability to work with people (staff, visitors and clients).
